The condition where either males or
females feel that they identify more as being of the opposite
sex is know as gender identity disorder. This disorder can
cause discomfort regarding their bodies and how they feel about
themselves. Many individuals who are classified as having this
disorder or belong to transgender groups choose to express
themselves as the sex that they feel they are and not the
biological sex that they were born as. This term is most often
used by physicians and physiologists.
Transgender groups are not always accepted in society. This
can cause the individuals to feel that they do not fit in
anywhere, because they are facing issues with how they feel in
their own bodies. For some individuals, the feelings of gender
identity disorder may have been there since early childhood,
while others may have not felt the first stages of gender
confusion until they hit their early teenage years or even in
early adulthood.
Many people who are transgendered do not really
see themselves as having a disorder. Some believe that
they have simply been born with the wrong body or as the
wrong sex. Those who are within certain transgender groups
sometimes question what the norm should be for a person
who is experiencing feelings of not having been born as
the correct sex. Many argue that gender identity disorders
are a product of social construction and are not related
to the biological sex or feelings of a person at all. Over
the years, there have been cures that have been reported
for this disorder, but the cures have never been
substantiated. There has also been the question as to
whether to offer counseling to help them deal with their
biological sex. It is very controversial as to whether or
not an adolescent or young child should be allowed to
explore their transsexual feelings. This has brought up a
lot of mixed feelings, both in transgender groups and in
mainstream society.
There are many individuals who do not accept those in
transgender groups. This can cause issues of feeling socially
withdrawn and of not knowing who they are. These problems are
many times present in these individuals already because they
are not confident in their own bodies. Gender identity disorder
is used by physicians to describe what they feel is a real
illness that needs to be treated. There are physicians who
specialize in treating both adults and children that feel that
they are part of transgender groups.

It's not really about "confusion" on the part of the character, as she appears fairly secure in her gender identity. It's those around her who are not so accepting. Family problems, sexual abuse, and drugs and alcohol also lead her to develop and act out behaviors that push others away.
When she is admitted into the mental health system as a young teen, she learns that there are manipulations and a merit/demerit system ruled by the powers that be. She learns who her enemies are, and forges fierce friendships that are tested and torn. Fellow "patients" accept her more than her own family.
The reader observes her character grow with each new discovery--whether a genuine spark of realization about herself, or a reluctant compromise on the part of a desperate soul willing to do nearly anything to escape her hell.
I found this book on the "gay and lesbian" shelf of a mainstream bookstore. It's refreshing to read such a memoir, and I highly recommend it to anyone in the LGBT community (and friends and family) who struggles with gender identity, or just feeling so darn different.
